23/03/2020 - UPDATE - COVID-19 - Information for visitors to Lakeland & Lunesdale Physiotherapy Clinic

Lakeland & Lunesdale Physiotherapy Infection Control Policy regarding:

CORONAVIRUS (COVID-19)

Advice from Public Health England:

At the moment, we are still open and operating our service in accordance with the advice we are receiving from the CSP (Chartered Society of Physiotherapy) and PHE (Public Health England). The following information has been produced in line with the advice from both PHE and the CSP.

The most common symptoms of Coronavirus (COVID-19) are:
• recent onset of a new continuous cough
• and/or high temperature (37.8oC or higher)

If you have these symptoms, however mild, regardless of age, or pre-existing health conditions, stay at home and do not leave your house for 7 days from when your symptoms started. You do not need to call NHS 111 to go into self-isolation. If your symptoms worsen during home isolation or are no better after 7 days, contact NHS 111 online. If you have no internet access, you should call NHS 111. For a medical emergency dial 999.

In line with this advice, we ask that patients do not attend the Physiotherapy Clinic if they develop symptoms of a new cough and/or a temperature of 37.8oC or higher for at least seven days from the onset of symptoms.

AFTER YOUR APPOINTMENT:
Please inform the Physiotherapy Clinic if you are diagnosed with COVID-19 within 7 days of attending the clinic.

Are you in the high risk category?
Patients over the age of 70, and those with certain pre-existing health conditions fall into this category, and will have been identified and notified by their GP surgery. Current guidance advises that patients who fall into the high risk category should restrict their contact with other people to an absolute minimum. Remote access to healthcare is being advised, where possible. Only clinically essential treatment is recommended. Therefore, we would strongly advise these patients not to attend for treatment unless it is unavoidable. Please can you telephone the clinic to discuss your individual case with a clinical member of staff. Where possible we will provide self-management advice over the telephone to help alleviate your condition.

We are hoping to put together some information to help you stay as strong and active as possible during periods of isolation.

Infection Control Policy at the clinic:
All visitors to the clinic must wash their hands with soap and water on arrival and on leaving. We have hand washing facilities in our bathroom, which can be used by visitors. Hand washing posters are displayed at all hand washing stations. Hand towels have been replaced by disposable paper towels.

If a visitor to the clinic would prefer to wait outside or in their vehicle, rather than using the waiting room then please do so. We can call you in when we are ready for you.

The clinic is following the infection control advice from PHE and CSP, such as hand washing after every patient contact; cleaning treatment couches and other equipment with appropriate clinical wipes after each patient use, and single use paper towel is used over our pillows. Door handles and surfaces are cleaned daily with clinical wipes. Office equipment is cleaned daily using clinical wipes. We follow strict clinical waste guidelines.

We will not charge for appointments which are cancelled due to development of COVID-19 symptoms though we would appreciate as much notice as possible when cancelling appointments.

Other advice for the general public to help infection control:
• Wash your hands more often than usual, for 20 seconds using soap and hot water, particularly after coughing, sneezing and blowing your nose, or after being in public areas where other people are doing so. Use hand sanitiser if that’s all you have access to.

• To reduce the spread of germs when you cough or sneeze, cover your mouth and nose with a tissue, or your sleeve (not your hands) if you don’t have a tissue, and throw the tissue away immediately. Then wash your hands or use a hand sanitising gel.

• Clean and disinfect regularly touched objects and surfaces using your regular cleaning products to reduce the risk of passing the infection on to other people.

If a patient wishes to wait in their vehicle prior to their appointment (and not in the waiting room) they should notify the clinic so that the clinician can be informed.

11/10/2019 - Have you had an ankle injury?

It is always worth getting checked by a Chartered Physiotherapist following an ankle sprain or an ankle fracture.

It is very common following ankle injury to lose range of movement in the ankle, especially dorsiflexion. This is the movement required to pull the foot/toes up towards the face and when weight bearing to bend the knees forward over the toes. This movement can become restricted by injury to the soft tissues around the ankle or by immobilisation in plaster following fracture/sprain, which leads to adaptive shortening of the calf muscles. The calf muscles have to stretch in order to get the knees to bend forward over the toes (a very frequent daily movement), and to heel strike when walking. If dorsiflexion is restricted then the body will be forced to compensate. Typically the foot will turn outwards, placing a twist on the knee joint. For example, if you’re trying to come down the stairs with a right ankle that won’t dorsiflex, when you step down with your left foot, the right knee has to bend forward over the toes, requiring dorsiflexion of the right ankle. If the ankle can’t do this the foot will turn out to compensate, creating a torsion at the bent knee while it is bearing the body’s full weight. Knees hate being twisted, and will become painful, worsening with time and repetition. When bending down in everyday activities the same will happen.

When walking or running, the same thing will happen. When the ankle is required to dorsiflex the foot will tend to turn out, more so as the calf becomes less flexible.


11/10/2019 - Ankle restriction causes knee pain

It is very common to find a turned out foot and tight calf at the root of many knee problems. The patient complains of pain coming downstairs and on any twisting/turning movements. On further questioning, it transpires they’ve had a nasty sprain in the past or a previous break to the ankle. It’s a very common cause of future problems, especially in the knee.

If the calf remains tight, and the mechanics at the ankle remain abnormal it will have a knock on effect right the way up the leg to the spine, potentially causing hip, pelvic or spinal pain.

So, if you’ve had an ankle injury, make sure you get checked by a Chartered Physiotherapist to ensure you have regained full range of movement, and haven’t developed any compensatory patterns. The Physiotherapist may need to work on releasing your calf muscle, and mobilising your ankle to help you regain dorsiflexion, and can instruct you on appropriate exercises to do at home.


11/10/2019 - Simple Exercises to do following an ankle injury:

Stand with your feet hip width apart, feet parallel. Bend your knees forward over your toes without allowing your heel to lift up, your foot to pronate (arch drop), or your foot to turn out. Keep your knees hip-width apart; make sure they don’t turn in as they bend forward. Repeat this movement little and often as soon as you’re able to fully weight bear.

Practise balancing on one leg to ensure you stimulate your balance reactions again on that foot so that when you walk on uneven ground the muscles that control the stability of your foot and ankle respond. This minimises the chance of you going over on it, and going back to square one! Once you can balance well on flat surfaces, practise standing on a cushion to simulate uneven surfaces.

03/09/2018 - Welcome!!

We would like to welcome two new members to the Physiotherapy Team:

Janet Evans MCSP has been with us for a few months now and is a valuable asset to the team. Janet mainly works on a Monday morning as she also has commitments within the NHS working as a Specialist Physiotherapist in the Musculoskeletal Outpatients Department. Janet has worked in the Department since 2000, based in Morecambe Bay NHS Trust.

Martin Grundy MCSP started working with us at the beginning of July and is a welcome addition to the team. He currently works as a Specialist Physiotherapist within Musculoskeletal Outpatients for Cumbria Partnership NHS Trust but we are lucky that Martin is also able to work with us on a Tuesday, and the occassional Saturday clinic.

Martin qualified in 2012 from Salford University via the Professional Footballers Association. He has worked in the NHS since he qualified, working in Barrow-in-Furness and Kendal. Within this time he has gained experience as a Junior Physiotherapist and Specialist Children's Physiotherapist in both acute and community settings covering the following areas; Orthopaedics, Respiratory Care, Surgical Care and Musculoskeletal Outpatients.

Martin has a keen interest in sports related injuries and rehabilitation. This varied experience has provided him with the knowledge to offer expert advice and management of complex musculoskeletal conditions.

Outside of work, Martin plays football at a semi-professional level for Kendal Town FC. His experience of playing football has given him more awareness of the how important strength and conditioning is to prevent injuries and the hard work required when returning from injury.

Please note that as of September 1st 2018 our professional fees for physiotherapy services will increase.

It has been a number of years since we have altered our fees and, due to increasing expenditure, it has become a necessary business decision.

We apologise for any inconvenience caused and look forward to continuing to provide you with an excellent standard of care.

Fees for Physiotherapy services as of September 1st 2018:

Physiotherapy initial assessment/assessment on new condition (approximately one hour) £55

01/10/2016 - Farewell Kerry!

We would like to wish Kerry Singleton best wishes as she embarks on a new chapter in her life.

Kerry left on maternity leave last year and had a beautiful little boy in December. Unfortunately (for us) but not surprisingly, Kerry has decided to rethink her working life and spend more time with family. Therefore Kerry won’t be returning to Lakeland & Lunesdale Physiotherapy Clinic.

We are very pleased to introduce Tony Bennett, a Registered Instructor with the Tai Chi Union of Great Britain. Tony has joined us to offer one-to-one training in Tai Chi & Qigong to clients.

Tai Chi Qigong is an ancient practice, taking us back to our human roots when feeling was our guide rather than our intellect. Reading our feelings, and responding in ways that change those feelings for the better, is a lost skill. Those people who developed Tai Chi Qigong worked to put together the elements of that skill and passed it on to others over at least 5000 years.

Tai Chi Qigong is a way of becoming more attuned to our physical bodies and our emotions to enable us to relax, taking tension out of our bodies and changing the way that we feel. Through taking control of ourselves, this has a direct impact on our body’s immune and healing systems and therefore our general and specific well-being.

We practice Tai Chi Qigong both sitting down and standing up, through being still and through movement. The movements are gentle and slow, taking the strain out of our muscles and joints.

Tai Chi Qigong can be practised by anyone, whatever their condition and ability. Its benefits range from easing stiffness, reducing or removing pain, increasing flexibility and ease of movement to bringing about greater peace and confidence. It complements conventional medicine and is particularly beneficial in managing chronic health conditions.

Tony has a Diploma in Stress Management Training, a qualification in Person-Centred Counselling and Certificate in Emotional Freedom Technique. He is a Registered Instructor of the Tai Chi Union of Great Britain.

Tony is currently offering hourly one-to-one sessions at the clinic which cost £35.

06/07/2016 - AXA PPP HEALTHCARE INSURED PATIENTS

AXA PPP Healthcare is changing their provider scheme.

Unfortunately, due to the new terms and conditions they are wishing to impose and the low fees they are willing to pay for our expert services, it is this practice’s considered decision to not enter into a new agreement with them.

We have until 31st July to treat EXISTING AXA PATIENTS, after which time we cannot claim treatment costs through AXA and are therefore unable to accept any new AXA patients as of 4th July 2016.

We can still provide AXA patients with the exceptional service they have always received, but unfortunately you would not be able to claim the costs through AXA.

We regret to inform you that if you do need to reclaim the costs of physiotherapy treatment through AXA, we will not be able to treat you after 31st July 2016, and you will have to source an alternative provider.

We apologise for any inconvenience caused and thank you for your understanding.

Cath is now a qualified Tuina practitioner!

I am very pleased to say that I have added a ‘tool’ to my bag! Over an eight month period starting last year I have trained in ‘Tuina’, Chinese Medical Massage Therapy, and have successfully passed the examination. I am enjoying practising this very effective treatment technique on my patients.

Tuina is very easily incorporated into physiotherapy treatments, and is used to release tight tissues especially muscle & fascial tissue, to mobilise the joints and to stimulate acupuncture points. Treatment is applied through a thin sheet, which helps keep the patient warm and allows for smoother techniques while protecting the patient’s skin. This seemed a little strange at first as I was so used to working directly in contact with the skin. However, this seems to work well. It is especially useful in treating a more elderly client who may have more delicate skin, and generally patients relax more as they feel warmer and less exposed! However I might add that this is not a relaxing massage, it is therapeutic and purposeful!!! The depth of Tuina can be adapted according to the patient and their needs. Tuina involves many different techniques, of which I have learnt over twenty.

Tuina is widely used in China and is still relatively unknown in the West. The oldest texts date back to 1405, so it’s stood the test of time! ‘Tui’ means to push and ‘Na’ means to grasp, and all the techniques contain an aspect of either Tui or Na. Chinese medicine believes that pain is caused by ‘blockages’ resulting in stagnation of Qi (energy) and Blood. To put this in a western context, it is well documented that tight myofascial tissue (this is the muscles and the fascia-the tissue that ‘vacuum wraps’ the muscles and organs) can trap nerves or blood vessels causing pain. Tightness (blockages) may be caused by muscle tension or scar tissue. If a muscle is in an abnormally contracted tight state, it squeezes the blood vessels depriving itself and other tissues of blood. Tissues can’t function without oxygen and nutrients supplied via the blood. This tightness (blockage)may also compromise the nerves passing through the muscle or adjacent to it. It may also pulls joints out of alignment; a benefit of Tuina documented by the Chinese is to realign tissues and joints. Some of the techniques are almost identical to our ‘physiotherapy’ techniques, and can help break down adhesions/scar tissue and stimulate collagen production/alignment (healing). It is a very rhythmical treatment which can help calm the Mind; people often report sleeping better after treatment. Chinese medical theory also believes that acupuncture points on the surface of the body when stimulated can help strengthen the immune system and help the health and balance of the internal organs.

The next couple of years………………

I am now just beginning a two year diploma in Traditional Chinese Medicine & Acupuncture to further my knowledge.

It’s a long story, but I have personally witnessed the remarkable effects achievable by this amazing medical system. Chinese Herbal Medicine has transformed the life of my youngest son dramatically and brought him from a state of misery & despair to a happy healthy bouncing boy. In searching for the answers to his troubles I have become infatuated by the subject. I have self-studied Traditional Chinese Medicine (TCM) over the last two years and my patients keep jumping out of the pages, as if they had written the chapter!

One example was a patient who came to see me with knee pain. I immediately observed that his leg was not in a correct alignment but was significantly turned out, a common cause of knee pain, as the knee is put on a twist. At the time he was telling me about the horrific migraines he had suffered which had required hospitalisation and had sent him delirious, and his high blood pressure. He also complained of a chronic sinus problem. I observed his excessively red face and eyes, and was concerned about his general health, his head looked ‘too full’, ready to implode. It was quite by chance that I was reading about the extraordinary meridians at the time, and I had never heard of the Yang Heel Vessel before. On reading the description of pathology affecting this vessel it was as if this chap had written the book. It described the pathway of the vessel going up the outside of the leg and eventually entering the head. It’s job is to ‘absorb excess yang energy’ from the head (I know this sounds weird!). When pathological, the leg turns out, and the head symptoms he had described will occur, and the clinician will observe a red face and red eyes. I was of course (unknowingly) already working manually along the Yang Heel Vessel, by releasing the fascia along the outside of the leg which anatomically turns the leg out when tight. I decided with his consent to add two acupuncture points one in his ankle and a one in the opposite hand which are said to ‘open’ this Vessel. I must admit I didn’t for one minute believe that two acupuncture points would have any effect….how wrong I was. When he returned and informed me that there’d been a significant change, I of course assumed he was referring to his knee, (which was better), but this is not what he was excited about..…..……………… His head symptoms have remarkably improved his face is now a rosy colour rather than a dangerous colour, and his sinus troubles have abated. His wife has also told me how dramatically his temperament has changed…he is less ‘hot-headed’ literally, which is visible! I can only assume that there is some mechanism whereby tightness in the leg creates some kind of back pressure to the head, via a fascial connection, but how these two needles dramatically improved that is quite mind boggling!! It makes me hungry to read more and more. I could also tell of patients with digestive disorders which seem related to their neck pain and stiffness, both these problems have responded very well to acupuncture.

My previous acupuncture training was very ‘westernised’ and I now realise didn’t even scratch the surface. Consequently it never really captured me and I only used acupuncture occasionally and without much conviction in the past. The subject of TCM is vast and very deep, and is based on thousands of years of very careful observations and trial and error, based on extremely detailed anatomical knowledge and physiological principles. Their descriptions are much more poetic and eloquent but are underpinned by strong consistent theories. Diagnosis is based on pattern recognition, they don’t rely on a western diagnosis or investigations. For example people with asthma are not given a ‘standard’ treatment, each individual is assessed and would be diagnosed and treated differently according to the pattern they present with. The tongue and pulse are used in every assessment/consultation as a diagnostic marker, in conjunction with careful questioning and other physical signs. TCM considers the body as an integrated whole which sits well with my way of thinking. I always want to know why, and I sometimes find our western medical model frustrating as it doesn’t seem to seek ‘cause’, rather it tries to ‘mask/suppress’ the symptom. The theories of Yin & Yang & the Five Elements are based on the laws of nature, and like it or not we are governed by natural forces throughout every day and night of our lives! Despite this being an ancient medical system it seems to me it already has explanations for our modern medical mysteries!

A word of warning!!

Unfortunately Chinese medical practitioners do not have a protected title in the UK. There are recognised professional bodies which qualified practitioners are registered with. Please ensure you only seek advice from a recognised professional. (British Acupuncture Council, Association Traditional Chinese Medicine / For oral herbal medicines make sure they are also on the Register of Chinese Herbal Medicine).

When I first qualified, the physiotherapy profession was no different. Back then anyone could call themselves a physiotherapist, and could pull people’s necks around without any knowledge or skill. This was dangerous and the public were vulnerable, as was our profession. Our profile was being jeopardised by these unscrupulous practitioners, (The profession of Chinese medicine is currently in a similar position). Now we have been granted protection of title of ‘Physiotherapist’ after fighting for it!

04/11/2015 - ALTERATIONS TO THE PHYSIOTHERAPY CLINIC

The layout of the clinic is about to change!!

We are going to change the existing waiting room into two new treatment rooms. One will be an extra physiotherapy treatment room, and the other will be used by Sarah Inman (BANT registered Nutritional Therapist), and possibly some additional professionals.

The new waiting room will be situated at the front of the current gym area, and the front door will be moved accordingly!

This work is starting this weekend and will be done in stages so as to try to cause minimal disruption to our usual operation.

However, please note that parking will be limited due to workmen needing to park their vehicles. Alternative parking can be found nearby at Booths or the public car park or on the Square.

We apologise for the unavoidable inconvenience while this work is carried out and hope you will appreciate the additional services we shall be able to provide.

12/10/2015 - New Pilates Instructor

We are very pleased to welcome a new 'Body Control Pilates' instructor, Nickey Russell, to our team.

Nickey qualified as a Physiotherapist in Sheffield in 1999 and has spent the last 16 years working around the Northwest in both private health care and NHS hospitals – currently at the BMI Lancaster hospital. Nickey predominantly works within pre and post-operative Orthopaedics.

Nickey decided to train with Body Control Pilates as she has benefited greatly from practicing it herself over the years. It has helped her post-operative patients too. The Pilates training and assessment that Nickey has undertaken, added to the depth of knowledge she has gathered as a physiotherapist, offers a huge number of options for breaking down/changing exercises to ensure that everyone gets the most from each session. The Pilates classes are run with a maximum of 8 participants. Nickey also plans to run one to one sessions with a progress plan and, from next year, weekend courses for both runners and golfers.

Nickey lives near Hornby with her husband Richard, a local GP; their two daughters aged 10 and 6 years old and a selection of dogs, cats and ponies! She is a long-term obsessive horse rider, has grade 4 in classical ballet and has now taken up competitive hill running.

22/05/2014 - Introducing Helen Towers of 'Relax and Just Be'

Reconnect through breath, mindfulness, meditation & movement

I am delighted and excited to announce that Helen Towers is to start running her ‘Relax & Just Be’ relaxation workshops and courses from our lovely exercise studio.

Helen has been practicing meditation and yoga for over 10 years. She has studied Buddhist philosophy at foundation level during this time and has taught meditation classes in Lancaster, Kirkby Lonsdale and Arnside.

Also having a keen interest in holistic therapies Helen has previously studied a diploma in Anatomy and Physiology and Body massage as well as leisure courses in Aromatherapy and Reflexology.

Helen has been recently studying with Breathworks in Manchester & having completed the Mindfulness for Health course is about to start their teacher training programme.

Helen is also doing a foundation course with and is a member of the British Wheel of Yoga, with a view to going on to teaching level, and is a Member of the Mindfulness Association, having completed foundation training.

Helen previously worked for a number of years for a charity providing respite for people with disabilities and their carers and has also worked directly as a carer.

To all my patients,

I would like to say how sorry I am to have been absent from work over the past few months, and that I have not been available to help you. I appreciate your understanding. I am sure my colleagues will have looked after you with the same degree of TLC!!

I am starting to do some clinic hours again, but on a limited basis, so would like to give priority to those patients who have previously been under my care, should they be in need of treatment.
I am hoping that my clinic hours will gradually increase over the next few months, as circumstances allow. I have allocated clinic hours in the diary, but please do leave a message if you cannot be accommodated, as I may have some flexibility on a week to week basis, and I will help you if I can.

18/07/2012 - Further challenges

Further to my previous post, I completed the Olympic Park run back in March. What an amazing experience, firstly viewing the stadium, but secondly running in the depths of the stadium, chariots of fire playing, before running out onto the track, and "sprinting" down the last 100m. Brilliant.
I have recently taken part in my first triathlon, well one leg of it as a relay team.I completed the run section, albeit not in any world breaking time, but "its the taking part" that counts! The event was great fun, not necessarily inspiring me to do a full one myself, but perhaps a sprint distance?
The next challenge is the Total Warrior event in a few weeks time. What is probably going to be a very muddy 10k, interspersed with crazy sounding obstacles! I wonder how I get talked into these events?!

On the 31st March, Kerry is participating in an exciting run, which takes in the Olympics parks for the 2012 Olympics, and culminates by crossing the finishing line in the athletic stadium! Exciting!
All I need to do is to return from my first skiing holiday next week unscathed.......

Cath is going to talk at the May NLBS members meeting in Hest Bank about horse rider related physiotherapy issues. This will cover the muscles necessary for good riding posture and muscle control. Tips will be shared on improving your muscle strength, muscle balance and muscle control. Cath will suggest the possible impact on the horse and rider by being out of balance!
Hand outs will be provided.
